ALTERNATIVES TO INSURANCE

Private Pay – option as an alternative to insurance.  The fee depends on the type of psychological service provided.

Sliding-Scale – is available to individuals who have modest incomes.  The rate for qualified     individuals is determined by income. A Sliding Scale form is completed by the client who then attaches a copy of a recent tax return.  If you would like the form before treatment please contact the Billing Representative below.

   OTHER FEES - Cash & Checks are accepted for payment, co-pays and deductibles.

   BILLING SERVICES - Joan T. Johnson, Billing Services, (508) 746-0540.  Joan will be available        to coordinate insurance coverage and obtain authorization for services.
